Look on the bottom right (as you are looking from the back) at the long shifting rod. There is a mounting piece (underneath and close to the axle) that stabilizes the rod. If that piece breaks so that it does not provide the stability that the rod needs, it will cause shifting problems. Make sure that the bearing (or maybe it is a brass insert, I can't remember) that the rod goes through in that piece is good. Just something to check.

What can happen from time to time is the shift rail heads can rotate on the shift rails. When this happens the selector finger can slip from the 2-3 shift rail into one of the other gates before effectively moving the 2-3 shift rail into the neutral position.
So what happens is the selector finger ends up "Stuck" in another gate (1-R or 4-5). At this point you are "stuck" in 2nd (reminds me of my 2002 season...long story).
When you go to remove the rear cover to inspect, everything can look normal if you are not paying attention to where the finger is located. What you will notice is if you place all the shift rails into neutral, the rails heads should all be about .030 apart from each other.
You will need a special tool to set those up. You could also take the bearing carrier and assembly to Todd Cook in Tempe (480.967.7901), and he can show you how to do it.

Shift rail heads rotated as Moses said most likely, Some cases the shift finger will will have excessive wear on the corners causing the shift finger to cross over into the next shift rail before the rail you where just in to reaches its neutral detent pocket . Causing it to pull back into that gear while the finger is in the next position.
